The parent company information is based on 2017 public records. The device classification information comes from FDA’s Product Classification by Review Panel, based on matches of recall data from the U.S. and Canada.

The storage bag used to ship the product in was labelled witha storage temperature range of 20-25c. the bottle inside is accurately labelled with the correct storage temperature range o f2-8c. each bottle is shipped in an individual storage bag with the following information: the product name catalogue number lot# and storage requirements. in this case the storage temperature was inconsistent and could cause incorrect storage of the product.
